Policies
Some insurance companies reimburse for Medical Nutrition Therapy, based on your medical diagnosis. Check with your insurance provider or review your insurance policy. Nutrition for Life will not file directly with your insurance company. Therefore, payment is expected in full at the end of each visit. Check or cash only. If you would like to file a claim with your insurance provider Nutrition for Life can provide you with all the necessary paper work.
Cancellations must be made at least 24 hours before your scheduled appointment or you will be charged the full amount for the appointment. If you fail to show for an appointment you will also be charged the full amount.
Minors will only receive counseling with prior consent from their guardian. If your minor is receiving counseling please accompany them to their first visit to fill out paper work. Please call Karrie to discuss her current rates and fees for service.
