Payment
Some insurance companies reimburse for Medical Nutrition Therapy, based on your medical diagnosis. Check with your insurance provider or review your insurance policy. Nutrition for Life will not file directly with your insurance company. Therefore, payment is expected in full at the end of each visit. Check or cash only. If you would like to file a claim with your insurance provider Nutrition for Life can provide you with all the necessary paper work.

Fee

Public Speaking Event (60min)……………………..…$150

Home visits may be a longer length of time than office visits.  For every 15 minutes over the initial 60 minutes $20 will be added to the final bill. Speaking engagements may also be longer than an hour.  For every 30 minutes over the initial 60 minutes $50 will be added to the final bill.

Cancellations
Cancellations must be made at least 24 hours before your scheduled appointment or you will be charged the full amount for the appointment. If you fail to show for an appointment you will also be billed the full amount.

Minors
Minors will only receive counseling with prior consent from their guardian. If your minor is receiving counseling please accompany them to their first visit.
